VA Nexus Letters & DBQs for Mental Health Claims

As a U.S. Army combat veteran and board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC), I provide independent medical opinions and Disability Benefits Questionnaires (DBQs) to support veterans’ VA disability claims for mental health conditions.

Common conditions I assist with:

  • Anxiety
  • Depression
  • PTSD
  • Panic disorder
  • Other service-connected or secondary mental health conditions

My Process:

  1. Initial Records Review + Telehealth Eval – $250 You upload your relevant records (C-file, service treatment records, buddy statements, etc.). I conduct a focused telehealth review. This fee is fully credited toward a Nexus Letter if I determine your case is supportable.
  2. Nexus Letter – $1,000 total ($750 additional) A detailed, evidence-based nexus letter with my medical opinion on service connection (“at least as likely as not”), including rationale, DSM-5 criteria, and reference to your records.
  3. Optional DBQ – +$100 I will complete the appropriate Mental Disorders or PTSD DBQ documenting current symptoms and level of impairment.

Most veterans choose the full package: Nexus Letter + DBQ for $1,100 total.

Important Notes:

  • This is a one-time independent medical opinion service (not ongoing treatment), unless you are a Maryland resident and need a new provider for mental health services (we accept insurance or self-pay).
  • I will only proceed with a Nexus Letter if the evidence supports it.
  • No guarantees of VA approval — the VA makes the final decision.
  • Turnaround time: Typically 7–14 business days after receiving all records and completing the initial evaluation.
